NEW STUDENTS:  Students new to Tantasqua/Union 61 must present proof of immunizations BEFORE they are able to register for classes AND a copy of a recent physical exam within 30 days.  These documents should be delivered directly to the health office to ensure student privacy.

 

CURRENT STUDENTS:  Students currently enrolled in the Tantasqua/Union 61 district are required to show proof of immunization OR a letter indicating religious or medical exemption.  All 11th grade students are required to provide a copy of a current physical exam and, beginning with the FY 2020-21 school year, proof of meningococcal immunization.

STUDENT ATHLETES/PERFORMING ARTS STUDENTS:  Student athletes and those enrolled in performing arts must provide proof of a physical exam annually (it must have been conducted within the previous 13 months).  Student athletes & those participating in performing arts will not be permitted to tryout if the physical exam is not current and uploaded to School Doc.
